By submitting, you agree to allow Cardinal Health to enroll you in the Generics program selection that you selected above, and you further agree to comply with the additional requirements set forth above that are applicable to your Generics Program selection. Your participation in these Generics Programs shall be subject to Cardinal Health's standard terms and conditions related to such Generics Programs and any agreement for the purchase of pharmaceutical and related products that you may have with Cardinal Health. You acknowledge that Cardinal Health is not acting as your agent with regard to any of the Generics Programs set forth above.
