The goal of this program is to prepare pharmacists to be solution-driven leaders in the quest for medication safety. At the conclusion of this symposium, the participant should be able to:

Discuss the nation's progress relative to patient safety since the IOM's report To Err Is Human and describe improvement strategies for the future.
Define medication reconciliation and describe the steps involved in the medication reconciliation process.
Discuss the importance of medication reconciliation in ensuring safe medication prescribing and the prevention of adverse drug events.
Describe challenges typically faced by organizations implementing a medication reconciliation program.
Describe a model for implementing medication reconciliation and accelerating improvement.
Discuss the importance of a multidisciplinary approach to a successful medication reconciliation program.
Describe the importance of the medication reconciliation review and tracking of performance over time.

Educational Review Systems is approved by the Accreditation Council for Pharmacy Education (ACPE) as a provider of continuing pharmaceutical education. Individuals may obtain up to 3.5 hours of credit or 0.35 Continuing Education Units (CEUs). Certificates will be issued to participants attending the entire program and submitting the required evaluation form. Certificates will be mailed within four weeks of the program date.
