Alaris® System (a.k.a. Medley™ System) recalls

Recall information regarding the Alaris® Pump module (a.k.a. Medley™ Pump module) related to Occluder Springs

Cardinal Health has received reports of inaccurate flow rate with the Alaris® Pump module related to misassembled occluder springs during the manufacturing or services of the mechanism assembly.

Recall information regarding the Alaris® PC Unit (a.k.a. Medley™ PC Unit) and various Alaris® System modules (a.k.a. Medley™ System modules) related to Grey IUI Connectors, U9 Socketed IC’s and/or U19 Socketed IC’s

Cardinal Health has received reports of channel errors with the Alaris® System due to failures of:
  • Grey IUI Connectors on the Alaris® PC Unit (model 8000), Alaris® Pump module, Alaris® Syringe module, Alaris® PCA module, or Alaris® SpO2 module
  • U9 Socketed IC on display boards of the Alaris® Syringe module and Alaris® PCA module
  • U19 Socketed IC on the Alaris® Pump module logic board
